_____ I plan to attend The 28th O.D. World Congress at Cagliari Castle on the island of Sardinia, Italy. Enclosed is my registration fee of US $130 ______for members of The O.D. Institute and US $180_______ for nonmembers. (Checks must be drawn on a bank with a branch in the USA. The O.D. Institute does not accept credit cards.)

_____ I am a fulltime student and would like to apply for a scholarship. Enclosed is US $10 application fee.

_____ I will reserve my room at Hotel Califfo on the Island of Sardinia. A single room with breakfast will cost about US $78.30.
